Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. This role includes responsibility for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. The therapist serves as an educator to patients, families, students, coworkers, and themselves, while upholding professional standards and compliance with organizational code of conduct.

Responsibilities
  • Completes comprehensive evaluations of assigned patients, utilizing clinical reasoning to develop appropriate treatment plans and goals.
  • Ensures documentation is clear, concise, accurate, and timely in compliance with state, federal, and clinic guidelines.
  • Participates in student program as assigned, providing supervision and support to students.
  • Creates an individualized professional development plan and actively pursues continuing education and professional growth.
  • Adheres to discipline-specific State Practice Act and maintains professional licensure in good standing.
  • Implements organizational performance improvement initiatives and contributes feedback to maximize outcomes.
Required Experience / Certifications / Licensure
  • Degree in Physical Therapy from an accredited school.
  • 1+ year of outpatient clinical experience.
  • Current CPR certification.
  • Active and applicable state license for physical therapy.
