Pyramid Healthcare is seeking a compassionate and organized Case Manager to join our dedicated team in Moscow, PA. In this role, you will help coordinate care and support clients throughout their residential treatment journey by assisting with treatment engagement, aftercare planning, family communication, and coordination with community providers.

Job Summary

The Case Manager is responsible for coordinating care for clients and their families within residential treatment services. This includes providing individual and family case management support, facilitating educational and skill-building groups in collaboration with the clinical team, assisting with admissions and discharge planning, and coordinating services with outside providers and referral sources.

Essential Duties & Responsibilities
  • Coordinate client care through appointments, schedules, aftercare planning, and treatment team collaboration

  • Communicate professionally with referral sources, external providers, family members, and community partners throughout the client's treatment stay

  • Assist with utilization review (UR), legal coordination, employment-related needs, and other case management responsibilities

  • Arrange and oversee transportation needs for clients as needed

  • Conduct individual case coordination sessions to support client engagement and progress in treatment

  • Develop comprehensive aftercare and discharge plans, including referrals and outreach to continuing care providers

  • Support clinical programming by assisting with group sessions and maintaining consistent client engagement

  • Collaborate closely with counselors, clinicians, and interdisciplinary treatment team members

  • Maintain accurate and timely documentation in compliance with program standards and regulatory requirements

  • Work alongside the Client Navigation team to ensure smooth transitions into aftercare services

  • Provide onsite support for Enhanced Outpatient Sessions (EOS) between clients and the Client Navigation team

  • Participate in required corporate training, program training, and ongoing professional development opportunities

  • Perform additional duties as assigned

QualificationsEducation & Experience
  • High School Diploma or equivalent required

  • Minimum of one year of experience in a behavioral health, substance use, or mental health setting preferred

  • Valid driver's license preferred

  • CPR/BLS certification preferred

Knowledge, Skills & Abilities
  • Strong verbal and written communication skills

  • Excellent interpersonal and organizational abilities

  • Sound judgment and problem-solving skills

  • Ability to respond effectively in crisis situations

  • Knowledge of federal and state regulations related to behavioral health services preferred
