Job description
Job Title: Case Manager
Division/Department: Fatherhood PRIDE
Employment Type: Full-time
Employment Classification: Non-exempt
Essential Duties & Responsibilities: The case manager isresponsible for conducting community outreach to recruit fathers for theFatherhood PRIDE program, enrolling fathers into the program, providing casemanagement services, facilitating fatherhood workshop sessions in the community,and collecting evaluation data.
Specific Activities:
Fatherhood Workshops
- Facilitate fatherhood workshops including, butnot limited to 24/7 Dad, Inside Out Dad, Boot Camp for New Dads, Mom's Gateway,Growing Responsible Fathers, etc..
- Coordinate workshop schedule, location(s) guestpresenters, instructional materials, and supplies.
- Provide onsite support to outside facilitatorsthat provide financial literacy and workforce development.
Case Management
- Ensure fathers complete intake process to enrollinto program.
- Provide holistic and comprehensive case managementservices to all fathers including: intake and benefit assessments, goalssetting, long-term case-plan development, weekly case-plan development,progress monitoring, individual money management, tenant education, advocacy,and referrals.
- Engage regularly with the fathers in the program toensure that members complete program requirements, which include: parenting workshops, financial literacyworkshops, workforce development services, and case management services.
- Practice effective techniques for fatherengagement; demonstrate and encourage father friendliness within the agency andother community institutions.
- Maintain case-load to meet enrollment andcompletion numbers reported at team meetings.
- Monitor progress of participants on a regularbasis and document all interactions.
- Connect fathers with needed and availablecommunity resources.
- Follow-up with fathers and agencies asappropriate to document use/success of referrals.
- Input accurate and complete data for allcontacts with members into agency database.
- Ensure that all documents submitted on behalf ofa father are valid.
- Provide all required documentation in a timelymatter, which may include father follow-up outcome evaluation, client contactsheets, and evaluations.
Evaluation/Data Entry - Follow data collectionprotocols for reporting data; input accurate data within prescribed timeframes.
Program Special Events
- Plan, coordinate and implement quarterlygatherings for fathers in program.
- Plan, coordinate implement and attend specialevents like the Father' s Day Barbecue competition, Barbers for Babies, InfantMortality Awareness events, etc..
Administrative
- Attend all staff and team in-services,conferences, workshops and community meetings.
- Participate in grantee initiatives.
Community Outreach
- Promote community awareness regarding the valueof involved fathers.
- Conduct outreach to recruit fathers to enrollinto program at health fairs, community events, the court system, program specialevents, and through community partners.
- Conduct outreach to promote workshop scheduleand promote father attendance.
- Assist program director and program supervisoridentify, locate, and engage partners to refer fathers.
- Network within the community to provide casemanagement resources for fathers.
- Maintain relationships with current partnersthrough regular check-ins to see if they need referral forms, brochures, or ifthey have questions about the program.
Other- All other duties as assigned by the Director, FatherhoodProject.
Education and/orWork Experience Requirements:
- Bachelor's Degree in social services or relatedfield or equivalent work experience in the field.
- May substitute four or more years ofprofessional experience in community services and/ or counseling with high riskpopulations in non-profit sector for degree.
Additional Eligibility Qualifications:
- Ability to communicate both orally an in writingwith individuals for a wide variety of socio-economic backgrounds includingprofessionals, non-professionals, and the general public, concerning a widerange of subjects and /or situations.
- Excellent computer proficiency (MS Office Word,Excel and Outlook).
- Must be able to work under pressure and meetdeadlines, while maintaining a positive attitude and providing exemplarycustomer service.
- Knowledge of the criminal justice system andchild support court systems.
- Ability to work flexible traditional andnon-traditional hours.
- Ability to establish and maintain effectiveworking relationships with Coalition partner agency staff and the generalpublic.
- Ability to conduct discussions, interviews, andmeetings.
- Maintain professional boundaries.
- Experience in making public presentations and publicspeaking.
- Ability to maintain confidentiality.
- Experience working with diverse communities,organizations and agencies preferred.
- Knowledge of counseling principles andrecord-keeping techniques.
- Knowledge of community resources.
- Experience working with persons in crisis.
- Ability to work independently and to carry outassignments to completion within parameters of instructions given, prescribedroutines, and standard accepted practices.
- Must be able to communicate effectively and be ateam player at the Coalition.
- Must be able to commit to the Coalition visionand be passionate about its mission.
- Requires reliable transportation as travel isrequired, valid driver's license, good driving record, and automobile insurance.
Physical Requirements:
- Ability to safely and successfully perform theessential job functions consistent with the ADA, and other federal, state, andlocal standards.
- Must be able to travel to alternate locations.
- Regularly required to sit, stand, walk, speak,and hear while performing the duties of the job.
- The position requires extensive computer use sothe employee must have sufficient hand dexterity to use a computer keyboard andbe capable of reading a computer screen.
- Lifting/carrying up to 25 pounds.
