Job description

Job Objective:  

The Triage RN’s objective is to provide sound clinical communication and problem solving through a strong ability to triage on site situations, incidents, and staff questions remotely through both verbal and telephonic means and at a high volume of incoming calls.  Additionally, Triage nurses will be making outbound calls to Hospice, Home Care, Families, and updating provider portals via secured link. 

Qualifications: 

  • Must have a current RN license in good standing in the state of MN and any other state that ACS is providing services.  
  • Must have a minimum of 7 years of experience as an RN/LPN/CNA or combined in Assisted Living, Memory Care or LTC. 
  • Great organizations skills with the ability to hold self-accountable when not directly supervised. 
  • Exceptional communications skills both verbal and written  
  • Experience with Electronic Health Records i.e., Eldermark, Epic or Point Click Care, Residex and others. 
  • Experience or a willingness to learn state of the art phone and telecommunication technology 
  • Must have a good understanding of Pharmacology, Blood Glucose Monitoring, first aid,  high acuity LTC nursing and emergency preparedness.  
  • Must be professional and able to remain calm in difficult situations 
  • Must have a willingness to work with your team to manage time off and open shifts. 

Job Responsibilities: 

  • Answer calls when scheduled to be on duty including nights, weekends, and holidays. 
  • Must have great attention to detail and clinical investigative skills 
  • Document appropriately in the EHR 
  • Communicate professionally and effectively with a diverse workforce 
  • Must be able to follow and adhere to our work from home policies without variance 
  • Communicate professionally both in oral and written form while maintaining HIPAA complinace 
  • Promote a positive team approach and live our mission and core values for those who rely on your expertise 
  • Keep up with AL educational and mandatory state requirements.  
  • Self-Manage and track all various state licenses and CEU requirements 
  • Attendance of Mandatory Team Meetings/Training meetings outside of your work schedule.  

Physical Responsibilities (based on essential responsibilities) Must be able to frequently perform tasks such as, but not limited to those listed below: 

  • Must be able to communicate effectively via phone or telehealth.   
  • Must be able to remain awake and alert during overnight hours 
  • Must not consume alcohol or illegal substances while on duty or 12 hours prior to your shift 
  • Must be able to input information into an EHR 
  • Must be able to work every other weekend and every other Holiday 

Environment: 

  • This is a work from home position. You must have a quiet private space free from pets, family and background noise. 
  • You must maintain the appropriate internet bandwidth to support the applications needed to do the job. 
  • Travel needs to be reported to your manager for preapproval and internet verification.  Risk aversion to IT equipment.   
  • Must maintain permanent residence in MN, IA or WI or any other payroll approved state.  

Working Hours: 

  • Evenings and overnights M-F 
  • We offer a variety of shifts on Weekends and Holidays for 24/hour coverage based on client need and call volumes.
