Work Schedule:
60% FTE, day/evening, 12-hour shifts, every other weekend rotation plus required on-call responsibilities during the off-hours (evenings/nights) with a one-hour response time to travel to patient's home. Scheduled holiday rotation. This position is eligible for shift and weekend differentials and mileage reimbursement. Hours are based off the operational needs of the department. You will travel and work primarily in patient's homes located within a 30-mile radius of University Hospital; your assigned work location will be 301 S. Westfield Rd., Madison, WI.
Pay:
Additional components of compensation include impressive shift differentials - $4/hour evening, $5/hour night and $4.25/hour weekend differential as applicable for hours worked.

We are seeking a Registered Nurse (RN) to:
  • Provide hands-on acute care in the home setting, delivering hospital-level interventions including IV medication administration, lab draws, wound care, advanced assessments, and patient education.
  • Care for patients enrolled in the Home-Based Hospital Care program, utilizing technology such as remote patient monitoring, video visits, and mobile diagnostic services to support safe, high-quality care at home.
  • Collaborate closely with an interdisciplinary care team, including providers, program coordinators, paramedics, and support staff to ensure seamless, patient-centered care.
  • Perform comprehensive patient assessments, evaluating physical, functional, psychosocial, and cognitive status while monitoring response to treatment in the home environment.
  • Support safe transitions of care, identifying post-acute needs and coordinating referrals, services, and equipment in collaboration with the HBHC team.
  • Bring inpatient-level nursing expertise to an innovative care model, with preferred candidates having prior acute care or hospital-based nursing experience.

Qualifications
  • Bachelor's Degree in Nursing (BSN) Preferred

Work Experience
  • 2 years relevant acute care or home health nursing experience Required
  • Experience in care coordination, triage, or nursing leadership Preferred
  • Experience in program development Preferred
  • Experience in teaching self-care skills to patients and families Preferred
  • Recent, relevant home health experience Preferred
  • Experience using EPIC and virtual health platforms Preferred
  • Experience with IV therapy and knowledge of venous-access devices and ambulatory pumps Preferred

Licenses & Certifications
  • Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Required
  • Current CPR/BLS Certification Required
  • Current motor vehicle liability insurance in accordance with the UW Health "Motor Vehicle Policy" (Policy 1.17) and
    (1) A valid Wisconsin driver's license and
    (2) Successfully passing a driver's background check and
    (3) Two years of driving experience Required
  • Relevant certification Preferred
