Job description

Position Description:

Seeking an experienced Registered Nurse for PRN position with a home health care, Maryland residential service agency.

The Registered Nurse evaluates and advises regarding individual’s client health needs, and trains, monitors, and delegates nursing responsibilities to the Licensed Practical Nurse, Medication Technicians, Certified Nursing Assistants and Companions.

Duties and Responsibilities:

  1. Performs an initial assessment of the client to determine the individual’s needs in order to develop a plan of care. (Virtually or In Home Setting)
  2. Performs client Evaluation and Assessment at least every 120 days, and nurse supervisory visits, periodically, depending on the nature of the client’s condition, and the level of skilled nursing provided, including supervising caregivers at the client site.
  3. Oversees the administration of medications by observing each staff person administer medications at least every 30 days and reviewing the Medication Administration Records (MARs), at least monthly.
  4. Maintaining nursing standards and client services/care to a level that ensures client safety and quality of care. And complying with the professional code of ethics,
  5. Maintaining proper client documentation including updating client charts timely, and in accordance with HIPAA regulations. General Computer Skills.
  6. Observing and participating in Quality Improvement Programs and activities relating to the provision of care to the home health care client.
  7. Excellent communication and leadership skills. Fostering collegiality between all members of the health care team. Participating in appropriate meetings / committees, and actively participating as an "enthusiastic" member of a multi-disciplinary team.
  8. Demonstrating accountability and responsibility for your own actions.
  9. Practicing within the limits of own abilities and qualifications including, actively seeking information if unfamiliar with client diagnosis or any aspect of client care and actively seeking knowledge of any medications administered.

Qualifications:

  1. Must have an active registered nurse license issued by the Maryland Board of Nursing.
  2. Must have at least two year’s experience in providing skilled nursing services in the home health care setting.
  3. The use of a vehicle and a valid Maryland Driver’s license.
  4. Experience working with individuals with moderate to severe medical conditions in a home environment.
  5. Current CPR and First Aid certification.
  6. Available to respond to emergency calls, as necessary.

Other Requirements

  1. Successful completion of Maryland’s criminal background investigation.
  2. Obtain annual verification from a licensed medical provider that the individual is physically able to perform the requirements listed in this job description.
  3. Proof of Hepatitis B vaccination or declination required
  4. Obtain an annual PPD and TB test indicating that the individual does not pose a communicable risk to the public.
  5. Complete a skills assessment and competency test.
