Help Wanted: LPN/Case Manager – Assessment Nurse (EDWP Program)

About Us: We are a dedicated Medicaid waiver agency committed to improving the lives of elderly and disabled individuals through compassionate care and coordinated services. As we expand our reach and impact, we are seeking a skilled and empathetic LPN/Case Manager to join our EDWP team.

Position Summary: The LPN/Case Manager will conduct in-home assessments, develop person-centered service plans, and coordinate care for members enrolled in Georgia’s Elderly and Disabled Waiver Program. This role requires clinical judgment, documentation accuracy, and a strong understanding of Medicaid guidelines.

Key Responsibilities:

  • Perform initial and ongoing assessments for EDWP members

  • Develop and update individualized care plans based on member needs

  • Monitor service delivery and ensure compliance with waiver requirements

  • Collaborate with providers, caregivers, and interdisciplinary teams

  • Maintain timely and accurate documentation in accordance with state regulations

  • Educate members and families on available services and supports

Qualifications:

  • Current Georgia LPN license in good standing

  • Minimum 2 years of clinical experience, preferably in home health, geriatrics, or case management

  • Familiarity with EDWP, CCSP, or Medicaid waiver programs a plus
