Summary:

The Cardiology Service, in partnership with the Center for Advanced Practice at WMCHealth, are seeking an experienced cardiology Advanced Practice Provider (APP), Physician Assistant or Nurse Practitioner, to join the Heart and Vascular Institute at Westchester Medical Center in Valhalla, New York. The ideal candidate will have a strong desire to expand their clinical skills amongst a dedicated cardiac team with a proven record of excellence in the field of advanced heart failure. The APP should be able to work independently, demonstrating ability to multitask and work flexibly within a dynamic hospital-based cardiology practice.

Role Highlights:

  • Support our growing LVAD outpatient program including initial post-hospital evaluation, ongoing management, troubleshooting alarms, and act as a liaison for patients and their multidisciplinary team.
  • Conduct clinical visits for heart failure patients and support our heart failure physicians outpatient practice needs.
  • Maintain meticulous and timely documentation in compliance with institutional and regulatory standards.
  • Assist with data collection for INTERMACS (Interagency Registry for Mechanically Assisted Circulatory Support).
  • Participate in biennial Joint Commission surveys and attend weekly multidisciplinary team meeting.
  • Manage a cohort of patients with CardioMEMS pulmonary pressure sensor.
  • Assist with patient telephone messages and anticoagulation management.
  • Serve as an educational resource for patients, families, and interdisciplinary staff.

What Were Looking For:

  • Board certified Nurse Practitioner or Physician Assistant who possesses an applicable New York State license
  • Experience in general cardiology, heart failure and LVAD is preferred. Will consider exceptional candidates without LVAD experience.
  • Must possess excellent communication skills and demonstrate an ability to work as part of a multidisciplinary team.
  • Candidates with active BLS/ACLS certifications
