Job description

Overviewย ย Embark Behavioral Health is growing and seeking a Virtual Case Manager to join our telehealth-based IOP/PHP program serving teens, young adults, and their families. This role supports clients both clinically and administratively while partnering closely with therapists, leadership, and families to ensure coordinated, highquality care.ย Embark operates programs nationwide, providing a full continuum of behavioral health services-from residential and wilderness therapy to outpatient and virtual care. Our Virtual IOP program plays a critical role in expanding access to lifesaving mental health treatment.Responsibilities
  • Coordinate client schedules, attendance, and clinical documentationComplete education assessments,
  • Education Success Plans, and school communication
  • Serve as primary liaison for families, schools, and clinical team
  • Support admissions, pretreatment, case consults, and discharge planning
  • Assist with PHP/IOP scheduling, group facilitation, and case management
  • Manage consents, benefits verification, outcome measures, and EHR documentation
  • Coordinate medication management and access to health services
  • Participate in staff meetings, supervision, and assigned duties
Qualifications
  • Bachelor's degree in a social services-related field or equivalent experience (Master's preferred)
  • Experience in behavioral health and/or education strongly preferred
  • Strong organizational, communication, and timemanagement skills
  • Ability to remain calm under pressure, maintain boundaries, and resolve conflict
  • CPR/First Aid certification (or willingness to obtain) and ability to pass a background checkย
