American Traveler is seeking a licensed Physical Therapist with Home Health experience and OASIS proficiency for a 13-week travel assignment in Alameda or Contra Costa County, CA.
Job Details
• Home health setting serving patients in Alameda or Contra Costa County, CA,
• Day shift schedule, 5x8-hour shifts (9:00 AM - 5:00 PM),
• Weekends required on an as-needed basis,
• No on-call requirement,
• Uses Kinnser/WellSky EMR; training provided,
Job Requirements
• Minimum 1 year of recent home health PT experience required,
• OASIS experience required,
• Active CA PT license required; license must be in hand at time of consideration,
• Current BLS certification required,
Additional Information
• Responsibilities include physical therapy evaluation and treatment of patients in a home health setting across Alameda or Contra Costa County,
• 2 professional references from a supervisor within the past 2 years required for consideration,
• Full 7-year work history with all gaps explained required for consideration,
