Description

Purpose:


Ensure that community living services are performed at a level that meets or exceeds client and Agency expectations. This position supports the goals and objectives of the CILA by providing direct service support and administrative coordination to the clients who reside in the CILA. Coordinate the scheduling of service activities to ensure timely execution of client appointments. Review reports to ensure adequate supply levels are maintained.


Qualifications:


Two years of college or one year of college plus two years of full-time related work experience or high school diploma or equivalent plus five years full-time related work experience required. Must have proof of Direct Service Professional (DSP) training and pass TABE test within 120 days of hire. Must have a valid driver's license and ability to drive agency vehicles.

Requirements

Essential Responsibilities:


Evaluate and provide training to residents in daily living skills including, but not limited to: personal hygiene and grooming (which may involve physically assisting individuals to complete the tasks), housekeeping, laundry and clothing care, meal planning and preparation, money management and budgeting, socialization, and .recreation leisure activities.


Assist residents in achieving their maximum level of independent functioning in regards to goals stated in the Independent Habilitation Plan.


Implement group and individual programs which promote growth and development toward independent living skills for residents.


Assist residents in preparation of daily schedules to include performance of household chores, social activities, and other meetings and appointments.


Complete and insure the timeliness of all assigned documents and forms to inclu e daily progress reports, CILA shift reports, van log sheets, incident reports, MAR sheets, etc.


Monitor maintenance of living facility to assure safe, clean and healthy environment for individuals. Perform cleaning and housekeeping duties such as, but not limited to: vacuuming, dusting, sweeping, and mopping, monitor and assist residents with daily bedroom maintenance.


Monitor residents to insure wellness and safety, enforcing facility curfew if required.


Supervise resident's schedules and activities to insure timely arrival to day program or job site and other appointments. 


Supervise and assist residents in taking their medication in accordance with program guidelines.


Attend and participate in all IHP staffing's.


Assist in planning and supervising recreational and social program activities.


Plan menus and assist with meal preparation as required. Monitor and observe residents with special dietary needs.


Attend and participate in all staff meetings.


Respond to medical and behavioral emergencies following appropriate Health and Safety, Aggression Management, First Aid, CPR, and other procedures as necessary.


Monitor and assist residents with personal grooming and appearance as necessary.


. Immediately report any medical/behavioral issues or emergencies or other unusual incidents to Program Director, Associate Director and/or Division Director.


Other Responsibilities:


Transport and accompany residents to appointments and other services, including work centers, as needed.


Check all rooms upon arrival to insure that all residents are home and regularly monitored to insure wellness and safety.


Check and secure all doors and windows to insure building safety and activate alarm system nightly.


Perform other related duties as assigned.
