Wages

$23.00 - $24.00 per hour

Job Summary

COPAL is hiring a Wellness Navigator at their Minneapolis location to support community members by connecting them to essential services and resources — while honoring their lived experiences, culture, and autonomy.

Responsibilities

  • Conduct intake conversations with individuals seeking services in person at the worker center front desk, by phone, or virtually, prioritizing accessibility and choice.
  • Complete and maintain intake records and required documentation accurately, confidentially, and with respect for privacy.
  • Collaboratively assess needs, goals, and strengths to identify appropriate local resources and support options.
  • Connect individuals with community partners, agencies, and programs, including housing, healthcare, food access, legal support, and education.
  • Provide hands-on assistance with applications and documentation for services and benefits, as requested.
  • Offer follow-up support to promote successful connections, continuity of care, and meaningful outcomes.
  • Deliver culturally responsive, trauma-informed, and strengths-based support that honors lived experience and self-determination.
  • Work alongside COPAL’s navigator to triage needs, support service navigation, and coordinate appropriate solutions.
  • Build and sustain strong, collaborative relationships with local service providers, community-based organizations, public agencies, and local businesses.
  • Coordinate and facilitate presentations that share information about COPAL’s programs, services, and community impact if needed.
  • Participate in outreach events and community activities to strengthen visibility, trust, and community connection.
  • Contribute to data collection and reporting that supports collaboration, planning, continuous improvement, and equitable resource allocation.
  • Document services provided, referrals, outcomes, and exit information accurately and consistently in the Airtable database, in alignment with funder and evaluator requirements.
  • Maintain organized, timely, and confidential case notes and data records using Airtable and EveryAction.
  • Learn, understand, and articulate COPAL’s foundational values, mission, vision, and organizing principles, including the Jemez Principles and COPAL’s political platform.
  • Develop a clear understanding of one’s unit purpose and how individual responsibilities contribute to advancing COPAL’s broader mission.
  • Build proficiency in COPAL’s Customer Relationship Management (CRM) systems and technology tools relevant to the role.

Who You Are

  • You have a high school diploma or equivalent; an associate degree or relevant experience is preferred.
  • You have experience in social services, community outreach, case management, or nonprofit work preferred.
  • You are bilingual (Spanish/English), which is required.
  • You have previous experience in working with diverse populations.
  • You possess an understanding of, or commitment to learning about, progressive theories of race, gender, class, and intersectionality, and apply these frameworks to support equitable and inclusive engagement across diverse communities.
  • You have experience in group facilitation.
  • You can, after a month in the position, become proficient in the use of Airtable and EveryAction.
  • You can complete forms accurately and manage basic documentation.
  • You have strong attention to details.
  • Having knowledge of Twin Cities Minnesota community resources is a plus.
  • You can establish relationships and assist in individual and group conflict resolution.
  • You are willing to learn a variety of tasks; possess excellent communication skills, a thorough understanding of nonprofit practices, a serious work ethic, and a desire to work in a team-orientated environment.
  • You possess the ability to work independently, establish priorities, and meet deadlines.
  • You have strong organizational skills with the ability to manage multiple priorities.
  • You have strong computer skills and competency in MS Office.
  • You are comfortable working with diverse communities and building relationships with volunteers and partners.
  • You can work flexible hours, including evenings and weekends.
  • You can travel to sites within 15-mile radius of the office.
  • You have a valid driver’s license, access to reliable transportation, and the ability to meet COPAL’s driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
