JOB DESCRIPTION:
SUMMARY OF POSITION
Serves as an integral member of the multi-disciplinary team and a core facet in the continuum of care for Behavior & Primary Healthcare Coordination (BPHC)-enrolled clients. Provides individual, supportive coaching to improve physical health status; coordinates client care with team of internal/external providers; aids clients in accessing/utilizing resources; and promotes the best outcomes for our clients.
ESSENTIAL DUTIES & RESPONSIBILITIES
  • Provides individual, supportive coaching to improve physical health status for assigned clients, such as help in scheduling appointments with doctors, coaching on communicating more effectively with doctors and following instructions on medications or other doctor recommendations.
  • Coordinates client care with team of internal/external providers. Obtains Release of Information for all medical providers for coordination of care.
  • Provides life skills training, care management, and structured interventions to assist consumers in gaining access to resources and the skills necessary to improve their physical health status, quality of life, and self-management of symptoms. This includes areas like health hygiene, lifestyle habits, nutrition, reducing sedentary behavior, healthcare navigation.
  • Provides support across the phases of care that is responsive to the changing needs of the persons served.

QUALIFICATIONS
Education Level
BA/BS from a four-year college/university required.
Years of Experience
Two years of direct service experience working or volunteering in a human service organization.
