The Assisted Living & Wellness Director (LPN or RN required) is responsible for creating an atmosphere that provides for continuity of quality care and quality of life for all residents in the Assisted Living and Independent Living areas 24 hours per day. This is accomplished by providing clear direction and expectations to all partners about the philosophy of the area in which they work.

Responsibilities

  • Oversees daily operations of the Assisted Living including supervision of staff.
  • Completes Assisted Living 90 day and Annual evaluations.
  • Plans, evaluates, and takes responsibility for providing consistent and sufficient staffing to meet resident needs in the living areas.
  • Participates in recruitment and retention of partners.
  • Assists partners in utilizing appropriate techniques for effective assessment, planning, intervention, and evaluation of residents and their needs.
  • Set productivity expectations and hold partners accountable for meeting them.
  • Provide wellness services as indicated.
  • Ensures that the medical, psychosocial, emotional and spiritual needs of residents are addressed, followed up in the Plan of Care in an appropriate and supportive manner.
  • Ensures the area and its equipment and supplies are neat, clean, safe and utilized appropriately at all times.
  • Ensures campus and department policies, procedures, and guideline, as well as local and state regulations, are met and followed by everyone in the area.
  • Ensures adequate orientation provided to all partners in the area.
  • Participates with the interdisciplinary team by listening to concerns and suggestions and by providing follow-up and feedback in a timely manner.
  • Coaches and teaches partners how to communicate their needs as well as resident needs.
  • Ensures exceptional customer service provided.
  • Ensure the provision of resident physicals, drug screens and TB Screens
  • Schedule CPR and First Aide training for partners, track attendance to ensure compliance
  • Assist with partner scheduling and staff partners according to caseload
  • Provide work assignments to nurses and aides
  • Ensure partners complete assignments and scheduled
  • Conduct new resident assessments
  • Attend resident care conference meetings
  • Update incident reports to include interventions, write progress not reflecting interventions, enact interventions and sign incident report
  • Coordinate transitions in care from independent living to assisted living to long-term care
  • Attend meetings as scheduled
  • Attend daily morning health services leadership meetings
    • Report including details and analysis of any unplanned hospitalizations, ER visits and falls
    • Review progress notes and report relevant information to the team
  • Conduct daily partner huddle to relay pertinent, real-time information to staff
  • Meet with Director of Health Services weekly to establish common goals and evaluate progress toward set objectives
  • Complete shepherd visits weekly
  • Serve as on-call nurse in rotation with the Director of Nursing
  • Conduct periodic staff meeting to receive feedback from partners
  • Conduct periodic evening, night and weekend visits to stay connected to partners who work at these times
  • Develop processes and procedures for assisted living & wellness as needs arise
  • Communicate and build positive relationships and family members
  • Assist in the referral management process
  • Provides hands-on assistance in working the floor during periods of staffing challenges.

Qualifications

  • Education: Graduate of approved nursing program (LPN or RN)
  • Licensure/Certification: Current Ohio Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
  • Experience: Successful candidate will show strong organizational skills and system development, demonstrate effective communication, provide exceptional customer service and a commitment to quality care. The candidate will also need to demonstrate alignment with Otterbein Marblehead's mission and values. Previous experience in a continuing care community preferred.
    • 2 years healthcare experience preferred
