Social Worker/Admissions - Long Term Care

Part Time | Days | No Weekends

Pay: $24 - $35/hour (Based on Experience)


Masonic Villages of Pennsylvania - Warminster Campus - is immediately hiring a Social Worker/Admissions - Long Term Care.

What You Need as a Social Worker/Admissions - Long Term Care:

As a Social Worker/Admissions - Long Term Care, you will carry out our Mission of Love by being responsible for assisting the ID team in the planning, organizing, and implementing of the care plan and Social Services in accordance with current federal, state and local standards, guidelines and regulations, our established policies and procedures.

Our Social Worker/Admissions - Long Term Care is responsible for the following:

  • Assists the Admissions Director in the admission and inquiry process. Takes phone inquiries, guides facility tours, conducts resident/family interviews, contacts hospital social workers, facilitates decision making process of accepting the referral as an admission. Schedules admissions, meets with resident/family to complete paperwork and procedures, and works with other departments to assure smooth admission.
  • Performs social service assessments and histories, assists in development of a plan of care and ongoing evaluation of the resident's psychosocial and emotional needs.
  • Provides direct social work counseling and therapy, when indicated, for nursing home residents and families requiring emotional support for illness and placement related stress.
  • Maintains strict confidentiality of all sensitive information.
  • Documents resident adjustment to placement, progress towards meeting specific goals and significant behaviors in the clinical record at least quarterly or as needed. Maintains updated logs of documentation requirements and dates. Writes bed hold letters when resident is transferred to the hospital
  • Conducts confidential interview of the resident in any suspected abuse investigation.
  • Coordinates discharge planning efforts by communicating proposed and actual date of discharge, coordinating family resources, home health referral, Life Line, Meals-on-Wheels, equipment, etc.
  • Contributes to the interdisciplinary care planning process by helping to establish measurable and appropriate resident goals. Participates in resident care conferences. Interprets social, psychological and emotional needs of the resident/family to the attending physician and other resident care team members.
  • Participates in all mandatory facility in-services and others as appropriate to the position. Increases knowledge and keeps abreast of changes in the profession by reading current articles/books and/or participating in credited or non-credited educational programs.

What You Need as a Social Worker/Admissions - Long Term Care:

  • Bachelor's degree in Social Work (BSW) or other health-related or human services related degree (i.e. Sociology, Psychology, Gerontology, etc.)
  • Interviewing skills - ability to demonstrate empathy and understanding and ability to put others at ease.
  • Ability to communicate effectively with referral sources, medical staff, family members residents, and other staff.
  • Computer knowledge - i.e., word processing, MDS completion.
  • Experience working with adults and/or geriatrics preferred.
  • Long Term care experience required.
