Now Hiring: Social Worker

Social Worker Key Responsibilities:

- Conduct comprehensive psychosocial assessments to identify residents' needs and develop care plans.

- Provide individual and group counseling to residents and families to address emotional, behavioral, and adjustment issues.

- Coordinate with interdisciplinary team members to develop and implement resident care plans.

- Assist residents and families in accessing community resources, financial assistance, and support services.

- Advocate for residents' rights and ensure their preferences are respected in care planning and decision-making.

- Facilitate resident activities and programs to promote social interaction and engagement.

- Document assessments, interventions, and outcomes accurately in resident records.

Social Worker Qualifications:

- Master's degree in Social Work (MSW) from an accredited program.

- Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) in the state of Pennsylvania.

- Previous experience in a healthcare or nursing home setting preferred.
