Social Worker serves as an advocate to the Resident in a person center directed culture.  The social worker is a part of the interdisciplinary team, responsible for identifying and developing care plans that address the psycho-social needs of the resident/patient.  For assigned residents/patients, insure appropriate care plans, act as resident/patient advocate, staff education as appropriate.  Develop goals for residents/patients and work with residents/patients to achieve these goals.  Acts as liaison and provides educational support to residents/patients and their families.

Qualifications

Social Worker qualifications include:

  • Current License under the Social Workers, Marriage and Family therapists and Professional Counselors Act, or a Bachelors, masters or doctoral degree from an accredited school or program in social work or social welfare.
  • 1 year experience in health or related field.
  • Strong written and verbal communications skills.
  • Working knowledge of computers, Microsoft Word and Excel required.
