Job Summary:

We are seeking a compassionate and experienced Social Worker to join our long-term care team at our skilled nursing facility in Providence, RI. As a Social Worker at AdviniaCare Providence, you will have the opportunity to make a meaningful difference in the lives of our residents and their families. Our facility is committed to providing high-quality services that prioritize the unique needs and well-being of each individual.

Responsibilities:

  • Assists residents and their families in connecting with community resources, healthcare providers, and other social services

  • Conducts assessments and evaluations to identify psychological, financial, religious, and physical needs

  • Develops and implements individualized care plans that meet the unique needs of each resident

  • Collaborates with other disciplines to ensure comprehensive care and support

  • Provides counseling and support services to residents and their families

  • Maintains accurate and detailed records of resident interactions and services provided

Requirements:

  • Education: Bachelor's of Science in Social Work or a related field

  • Experience: Minimum 2 years of experience as a Social Worker

  • License: LSW, LCSW or LICSW preferred but not required
