Job Summary
The Social Services Coordinator is responsible to plan, organize, develop and deliver all required social services to meet the needs of our members and their families/caregivers. The Social Services Coordinator will be responsible for all Social Service Assessments and Summaries as well as provide case management services to address the psychosocial and socio-economic needs of members and their families or caregivers. The Social Services Coordinator communicates regularly with the Center Director and clinical team to ensure members receive the most comprehensive and quality of care possible and that all care is conducted within HIPAA regulations and in accordance with all state required laws and regulations.
Qualifications
  1. Must be licensed or certified by the New Jersey State Board of Social Work Examiners, pursuant to N.J.S.A 45:15BB-1 et seq.
  2. Minimum one (1) year social work experience in a health care facility, preferably working with seniors and/or adults with disabilities strongly preferred.
  3. Knowledge of applicable state and federal environmental and OSHA regulations
  4. Extremely strong collaborate skills across all levels of staff
  5. Excellent verbal and written communication skills
  6. Excellent interpersonal and customer service skills
  7. Excellent organizational skills and attention to detail.
  8. Excellent time management skills with a proven ability to meet deadlines.
  9. Thrives in a collaborative and team environment
  10. Good Computer skills
  11. Ability to step in and help the team as needed to support our members.

Job Responsibilities
  1. Provides case management to coordinate with internal and external ancillary services for members and families.
  2. Coordinates with Nurse Manager and/or members of the clinical team in the development and implementation of member plans of care.
  3. Participates on Safety and Quality Assurance Committees and establishes and schedules Member Council meetings, Caregiver Support Groups. Participates in Care Plan for Interdisciplinary Team and records clinical notes as required.
  4. Evaluates members referred to the Center to determine appropriateness for admission. Assists with referrals to other agencies when a member is not appropriate or becomes inappropriate to participate as a member at the Center.
  5. May explore and evaluate financial situations and assist members in using appropriate resources.
  6. Ensures compliance with all company policies and procedures and applicable regulatory authorities (ie: State Department of Health and Senior Services, state and federal environmental agencies, OSHA, USDA) and prepares paperwork as required.
  7. Maintains formal written communication with members and families and facilitates counseling through support groups and one-on-one sessions.
  8. Maintains accurate files and records of member and social services information. Provides billing, transportation and member referral information as needed.
  9. Participates in community outreach programs, including communication with other health and social service providers and inquiries from prospective members. Participates in Center marketing activities.
  10. Provides active assistance with members as needed (ie: hygiene, meals, ambulation, arrivals and departures, and social activities).
