Social Services Specialist - Fulltime

What You'll Do
  • Advocate for resident rights, preferences, and overall well-being
  • Communicate with families (with resident consent) regarding care updates and discharge planning
  • Coordinate referrals and services such as community resources, financial assistance, and supportive programs
  • Assist residents with financial and legal processes (Social Security, Medicare, Medicaid, VA benefits, etc.)
  • Build strong relationships with residents and collaborate with staff to support individualized care
  • Participate in interdisciplinary care plan meetings and help develop effective care strategies
  • Address and help resolve resident or family concerns and grievances
  • Complete social service assessments and discharge planning documentation
  • Maintain accurate, timely records in the electronic medical system
  • Provide guidance and oversight to social services team members as needed
