Job Summary

The Social Service Assistant at the Southern Utah Veterans Home provides essential support to the social services team by assisting residents with their social, emotional, and daily living needs. This role involves working closely with veterans, their families, and interdisciplinary staff to ensure a high quality of life and access to necessary resources.

Key Responsibilities
  • Assist social workers in conducting resident assessments and maintaining accurate documentation.
  • Provide direct support to residents by facilitating social activities and promoting engagement.
  • Help coordinate resident services, including transportation, appointments, and community resources.
  • Serve as a liaison between residents, families, and staff to communicate needs and concerns.
  • Support discharge planning and follow-up services as directed by social services staff.

Qualifications
