Work Environment

Key Responsibilities

• Conduct thorough assessments of clients to identify their social, emotional, and environmental challenges, providing insights that inform service plans.
• Develop personalized service plans tailored to individual goals, outlining specific interventions and methods for progress monitoring.
• Provide compassionate counseling support to clients and families facing a myriad of life challenges, utilizing evidence-based therapeutic methods.
• Facilitate crisis intervention processes, conducting risk assessments and creating safety plans to ensure client well-being.
• Collaborate with clients to identify and connect them with appropriate community resources, ensuring they gain access to essential services.
• Engage in community outreach initiatives to raise awareness of the services we offer and collaborate with community agencies for coordinated care delivery.
• Participate actively in case conferences, team meetings, and professional development training to enhance service delivery and community impact.

Patient Population / Client Focus

In this role, you will engage with a diverse clientele, including individuals and families facing issues such as homelessness, domestic violence, substance abuse, and poverty. Qualifications

• A Master's degree in Social Work from an accredited program is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits
