Key Responsibilities
  • Conduct comprehensive assessments of clients to identify their social, emotional, and environmental challenges, ensuring a thorough understanding of their unique situations.
  • Develop and implement individualized service plans in collaboration with clients, focusing on empowering them through achievable goals, interventions, and ongoing progress evaluation.
  • Provide therapeutic counseling to individuals and families facing diverse life challenges, utilizing evidence-based practices tailored to each clientโ€™s needs.
  • Engage actively in crisis intervention, offering immediate support, risk assessments, and safety planning when clients experience emergencies or acute distress.
  • Work collaboratively with community agencies to coordinate services, advocate for client needs, and facilitate smooth transitions into supportive systems.
  • Participate in community outreach initiatives to enhance awareness of the agencyโ€™s services, enrich community ties, and promote well-being among diverse populations.
  • Engage in regular case conferences and team meetings to collaborate with colleagues, enhance service delivery, and maintain high standards of practice.
  • Participate in ongoing professional development and staff training initiatives to stay abreast of best practices in social work and therapeutic interventions.
Patient Population / Client Focus

In this role, you will work predominantly with individuals experiencing serious life challenges, including mental health issues, substance dependencies, and socio-economic hardships. Our clients are diverse, representing various backgrounds and circumstances, each requiring tailored, empathetic support to navigate their obstacles and empower their self-sufficiency.

Qualifications
  • A Masterโ€™s degree in Social Work (MSW) from an accredited program is required.
  • An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory for this position.
  • Strong understanding of trauma-informed care principles and social justice issues.
  • Exceptional communication skills and a demonstrated ability to engage culturally diverse populations.
  • Strong analytical and problem-solving capabilities, enabling you to develop effective solutions for complex client issues.
Preferred Experience
  • Prior experience in a social service agency or similar setting is highly advantageous.
  • Familiarity with case management best practices, community resources, and advocacy frameworks.
  • Experience in crisis intervention techniques and safety planning.
Compensation and Benefits
