Key Responsibilities

• Conduct thorough assessments of clients to identify social, emotional, and environmental challenges, ensuring a comprehensive understanding of their situations.
• Develop personalized service plans that outline specific goals, interventions, and methods for progress monitoring tailored to each client’s needs.
• Provide expert counseling and emotional support to individuals and families, helping them navigate through difficult situations.
• Collaborate closely with clients to create individualized plans that connect them with essential community resources, enhancing their access to needed services.
• Offer crisis intervention services, conduct risk assessments, and develop safety planning strategies for clients in high-stress situations.
• Partner with community agencies and organizations to facilitate coordinated services and advocate for appropriate client support.
• Engage in community outreach activities to raise awareness about agency services, promoting active participation and resources available to vulnerable populations.
• Participate in regular case conferences, team meetings, and ongoing staff training, contributing to a culture of continuous improvement within the organization.

You will serve a diverse range of clients, including individuals and families dealing with significant life challenges such as mental health issues, poverty, domestic violence, and substance use disorders. Qualifications

• A Master’s degree in Social Work from an accredited program is required.
• Must hold an active state license as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits
