Position Summary:


The Certified Nursing Assistant is an integral role to the nursing team and has a significant impact on the overall delivery of care to facility customers. The CNA is responsible for providing quality care to customers and supporting the goals for shift objectives to enhance team efficiency and customer satisfaction.

Essential Duties and Responsibilities
1. Provide and assist with physical care of patients which will include but not limited to bathing, oral hygiene, grooming, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation and range of motion

2. Responsible for real-time documentation of patient care and observations

3. Communicate timely with Licensed Nurse any observed or perceived changes in patient’s condition or behavior

4. Assist in the dining process by preparing customer for meals; serves and collects trays, assists in setting up and feeding residents, serves refreshments. Completes meal assessment and records oral intakes accurately

5. Responsible for patient transport to scheduled therapy appointments

6. Assists in recreational, motivational and socialization activities of care program

7. Responsible for timely response to call lights and providing appropriate response or service

8. Responsible for regular rounding with customers to anticipate and identify needs and respond timely and appropriately

9. Fully understand all aspects of patient’s rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting incidents of evidence of patient abuse or violation of patient’s rights

10. Maintain the comfort, privacy, and dignity of each patient in the delivery of services to them. Encourages growth and independence as care progression warrants

11. Assists in maintaining the cleanliness and safety of customer room. Assists in maintaining a safe customer environment

12. Adhere to infection control procedures including but not limited to hand washing and isolation practices

13. Adhere to Par Scanning procedures to ensure accurate tally of supple usage and availability

14. Ensure uniform and personal appearance is clean and professional

15. Maintain clinical competence through completion of annual continuing education requirements

16. May participate in interdisciplinary customer and service improvement initiatives

Required Education/Experience

  • High School Diploma or Equivalent

Required License/Certification

  • Successful completion of a state approved Nursing Assistant Certification course
  • Valid state CNA license
  • Current BLS (CPR) certification

Preferred Education/Experience

  • A minimum of one (1) year experience in a CNA role
