Job Description

  • RN Hospice Case Manager- Per Diem
  • Performs comprehensive nursing evaluations.
  • Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action.
  • Initiates and manages plan of care, provides appropriate nursing follow -up to all patients, and instructs caregivers in managing patient needs.
  • Coordinates all services provided to patients and families, as well as appropriateness of care, to ensure quality of care and services.

Job Requirements

  • Education: Graduate of an accredited school of nursing.
  • Credentials: Current RN License within CA. Bachelors in Nursing preferred. Certification in Hospice and Palliative Nursing desired. A current CPR Certification, Valid Driver's License and auto insurance.
  • Experience: Minimum of one year experience as a professional nurse within the last 3 years with strong clinical assessment skills.
  • Skills: All nursing functions, written and oral communication skills, sympathetic attitude, customer services/relations, ability to work with dying patients, organizational and diplomacy.
  • Supervises: Provides supervision of Home Health Aides, Licenses Vocational Nurses, Licensed Practical Nurses, and Homemakers to ensure the best quality of care for patients.

Pay Range: $50 - $60 per hour
