Who You Are

  • The position reports directly to the Director of Health and Wellness and serves as an integral member of a collaborative team that provides support to a small community of deaf and hard of hearing adults with development disabilities. This role assists in coordinating all aspects of health care and wellness management, as well as staff training, records maintenance, medication administration, needs assessment, case management and recommendations for members' care plans. Familiarity with American Sign Language is a plus, but this skill may also be acquired on the job.

Key responsibilities include:

  • Provide nursing for multiple residential group homes and day program.
  • Conduct health assessments and on-going nursing evaluations.
  • Administer and monitor medications, treatments and physician orders.
  • Manage complex medical needs. (e.g. g-tube, seizures, mobility needs, chronic
  • conditions.)
  • Develop and update nursing care plans and health protocols.
  • Train and delegate nursing tasks to Direct Support Professionals.
  • Rotating on-call nursing support and clinical guidance for staff.
  • Coordinate medical appointments, labs and specialty care.
  • Communicate with physicians, guardians and interdisciplinary teams.
  • Ensure compliance with state regulations (BHDDH, Medicaid and agency policies.)
  • Participate in incident and quality assurance processes.

Preferred Skills:

  • Strong clinical judgement and independent decision making.
  • Experience supervising LPN and DSP staff.
  • Excellent documentation and care coordination skills.
  • Able to work autonomously in a community setting.
  • Compassionate patient centered approach.

Schedule:

  • Day shift, part-time, flexible hours.
