Job description

Admission Nurse/Evening Supervisor

Oakdale Rehabilitation and Skilled Nursing Center, part of Oriol Health Care, wants to help take your nursing career to new levels! We are currently seeking a dedicated and passionate working Admission Nurse/Evening Supervisor to deliver exceptional care, provide clinical and administrative leadership in the facility. This individual would be accountable and responsible for resident care within the facility and staffing coordination between all shifts. Schedule: 32-40 hours, 12:30pm-9:00pm, Monday-Friday with weekend/holiday on call rotation. Pay Rate: LPN Supervisor: $35.50-40.78/hr (with shift differential) RN Supervisor: $39.50-47.21/hr (with shift differential)

Sign-on/ Retention Bonus - up to $5,000 Skills: Strong clinical assessment and documentation skills. Knowledge of wound care, infection control, and admission process. Excellent communication, problem-solving, and organizational skills. Proficient in EMR systems and Microsoft Office (Word, Excel, Outlook).

Essential Functions:

Admission & Clinical Oversight

  • Coordinate and complete resident admissions, readmissions, and transfers.
  • Complete admission assessments and obtain necessary admission documentation.
  • Verify physician orders, medication lists, treatment plans, and required ancillary services upon admission.
  • Ensure accurate medication reconciliation and communication with pharmacy providers.
  • Orient residents and families to facility services, policies, and procedures.
  • Communicate admission information to nursing staff and interdisciplinary team members to ensure continuity of care.
  • Provide clinical and administrative supervision of nursing personnel during evening shifts.
  • Monitor resident care delivery to ensure compliance with facility policies, procedures, and standards of nursing practice.
  • Respond to changes in resident condition and coordinate appropriate interventions, physician notifications, and follow-up care.
  • Conduct routine rounds throughout the facility to assess resident safety, staff performance, and environmental concerns.
  • Serve as a clinical resource and mentor to nursing staff.
  • Assist with staffing coordination, assignment adjustments, and call out coverage as needed.
  • Investigate incidents and accidents and complete required documentation and follow-up reports.
  • Ensure timely and accurate completion of clinical documentation and shift reports.
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members.
  • Assist with implementation of infection prevention and control practices.
  • Maintain survey readiness and ensure compliance with federal, state, and facility regulations.
  • Assist with booking appointments and coordinating transportation for resident care needs.
  • Provide direct resident care as needed.
  • Perform other duties as assigned by the Director of Nursing.
