Care Needs & Planning: Assist to implement & revise care plans, monitors health conditions, supplements direct care needs & responds to emergencies or changes in status

Staff Oversight & Scheduling: Observes care assignments, manages coverage needs, evaluates staff performance, educate & lead staff

Family & Team Liaison: Communicates changes in resident conditions to families/providers, works collaboratively with the interdisciplinary team

Documentation: Reviews and investigates incidents, transcribes orders, audits medical charges and maintains record compliance for accuracy

Family, Resident, Staff Education: Explain health status, changes and expectations for care, guides and direct care skills with intervention as needed
