Key Responsibilities

  • Maintain open communication with supervisors, your colleagues, and clients' families to document progress and adjust treatment as necessary.
  • Schedule and coordinate appointments, ensuring continuity of care and effective follow-up through home visits and consultations.
  • Facilitate Interdisciplinary Team Meetings, contributing valuable insights and collaborating on client care strategies to ensure holistic treatment approaches.
  • Perform regular chart reviews and data analysis to monitor client progress and adapt treatment interventions accordingly.
  • Lead training sessions for staff members to ensure the integrity and adherence to the program plan and best practices in documentation.
  • Act as a liaison within the mental health community to foster relationships and collaborate on community initiatives aimed at improving mental healthcare accessibility.
  • Consistently document interaction notes, treatment progress, and outcomes to maintain accurate records for client care.
  • Engage in ongoing professional development to stay abreast of advancements in counseling and mental health practices.
Patient Population / Client Focus

You will work closely with a diverse group of clients, including adults and youth facing various mental health challenges such as anxiety, depression, trauma, and relationship issues. Qualifications
  • A Masterโ€™s or Doctoral degree in Counseling, Psychology, or a related field is required.
