Job Summary and ResponsibilitiesSchedule: Monday-Friday (Flexible - between the hours of 730a-5p)
**This position will be working remote after 2 weeks of IN PERSON training. MUST live within the Omaha Metro - Council Bluffs Location to be considered!**The Virtual Discharge Clinic RN facilitates inbound and outbound telephonic and computer communication with clinics, hospitals, patients, providers and consumers to deliver patient care that promotes wellness and safety.
- Discharge documentation, updating charts in the EHR system and coordinating follow-up appointments as needed
- Reviewing discharge instructions ensuring the patient understands next steps
- Medication review with patients as needed
- Provides virtual patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
- Accepts inbound communication such as calls from patients, pharmacies and other health-care entities.
- Performs outbound calls to notify patients of reviewed lab and imaging results according to protocols that require review of the patient chart to answer questions.
- Performs telephone triage utilizing evidence based triage protocols.
- Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.
- Follows medication refill protocols and acts accordingly to those protocols.
- Ensures accurate patient documentation.
Job RequirementsMinimum Qualifications:- Current Registered Nurse (RN) license that allows you to practice in our state
- Graduate of an accredited nursing program
- Basic Life Support (BLS) from the American Heart Association required
