Position Overview

Gotham Enterprises Ltd is seeking a skilled and compassionate Licensed Clinical Social Worker (LCSW) to join our dedicated team at our remote Community Center, located in Watertown, Tennessee. This full-time position primarily focuses on improving the quality of life for seniors, providing critical support and guidance to enhance their well-being. As a Social Worker in this role, you will be instrumental in assessing the psychosocial needs of seniors, crafting personalized care plans, and ensuring seamless access to community resources.

Work Environment

This position offers a flexible remote work setting, enabling you to successfully engage with clients and their families while ensuring your work-life balance. Although primarily operated from home, occasional visits to community facilities or client homes may be required to deliver hands-on care and assessments. Your standard working hours will be Monday through Friday, from 8:00 AM to 4:30 PM; however, you should be prepared for occasional evening or weekend hours based on client needs. Flexibility in scheduling is essential to fulfill visits and provide continuous care. Additionally, being on-call may be necessary for urgent situations.

Key Responsibilities

• Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social elements that affect their overall well-being.
• Collaborate closely with clients and their families to develop tailored care plans that reflect individual goals and preferences.
• Provide counseling and emotional support, guiding seniors and their families through various life transitions and challenges.
• Advocate for clients' rights, ensuring they have access to community resources and services essential for their support.
• Work in collaboration with interdisciplinary teams, including healthcare professionals, to coordinate holistic care for clients.
• Conduct regular visits and check-ins to monitor clients' conditions, assess safety, and address any emerging concerns.
• Facilitate effective communication among family members involved in the care process to resolve conflicts and provide support.
• Educate clients and families about available community resources, support groups, and programs that enhance social engagement.
• Maintain meticulous documentation of assessments, progress notes, care plans, and other relevant client interactions.
• Stay informed about best practices in social work and current policies to uphold ethical standards.

Patient Population / Client Focus

Your primary focus will be on seniors and their families, addressing an array of psychosocial challenges unique to this population, such as isolation, mental health issues, and navigating healthcare systems. By actively listening and providing tailored support, you will play an essential role in enhancing their quality of life and promoting independent living.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• A minimum of two years of experience in a social work role, ideally in a community service or geriatric setting, is essential.
• Proficient knowledge of social work principles, aging-related challenges, and community resources that can benefit our senior clients.
• Demonstrated excellence in assessment, counseling, and problem-solving abilities.
• A passion for empathy, compassion, and rapport-building with seniors and their families.
• Strong interpersonal skills necessary for collaboration with interdisciplinary teams.
• Comfortable using electronic health record (EHR) systems and other relevant computer applications.

Preferred Experience

Experience working in geriatric social work or community-focused roles is favored. Proficiency in specific treatment methodologies and understanding of local resources available to seniors are plus points. An established network within Tennessee’s healthcare and social services is desirable.

Compensation and Benefits
