Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team at Consolidated Tribal Medical Services. Key Responsibilities
  • Conduct comprehensive assessments and therapeutic interventions, including individual counseling, psychotherapy, and care planning, to support clients and their families through their hospice journey.
  • Develop individualized treatment and discharge plans that reflect the unique needs of each client, assessing their emotional and psychological status through interviews, case histories, and observational methods.
  • Utilize evidence-based therapeutic practices and remain compliant with all regulatory and program-specific guidelines, ensuring timely and accurate documentation for all client interactions.
  • Maintain a current caseload of therapy clients, providing consistent follow-up and adjusting treatment protocols as needed, based on best practices and health plan regulations.
  • Engage in interdisciplinary collaboration with medical staff and other health professionals to coordinate comprehensive care, thereby enhancing overall patient outcomes.

  • Review and maintain comprehensive and organized client files, ensuring all documentation is complete, accurate, and meets state and federal standards.
Patient Population / Client Focus

This role primarily involves working with clients in a hospice setting, where sensitivity and understanding are key. You will address the emotional and psychological needs of individuals facing life-limiting conditions, providing compassionate support not just to the patients, but also to their families during difficult transitions. Qualifications
  • Masterโ€™s Degree in Social Work from an accredited graduate program.
  • Current and unrestricted Clinical Social Worker License (LCSW).
  • At least 2 years of experience in a healthcare setting, with a minimum of one year in a mental health capacity.
  • Strong clinical skills and experience in case management, social work, or related fields.
