• Conduct in-depth assessments to evaluate clients' social, emotional, and environmental challenges, tailoring interventions to their individual circumstances.
• Develop and implement personalized service plans with clear goals, interventions, and monitoring methods, working closely with clients to ensure their specific needs are met.
• Provide individual and family counseling, utilizing effective therapeutic techniques and approaches to navigate life's complexities.
• Engage in crisis intervention when necessary, performing risk assessments and facilitating safety planning to protect clients during critical moments.
• Act as a liaison between clients and community resources, enhancing their access to services that promote overall wellbeing.
• Collaborate with local agencies and organizations to coordinate care, advocate for client needs, and strengthen service delivery.
• Participate in community outreach initiatives aimed at raising awareness about available services and fostering engagement with the collective.
• Contribute to team discussions through case conferences and staff training sessions, continuously enhancing your own skills as well as those of your colleagues.

In this role, you will primarily work with clients facing complex life challenges, including those related to end-of-life care, grief, and loss. Your support will extend to individuals and families navigating difficult emotional landscapes, ensuring they have the tools and resources needed for resilience and healing during vulnerable times.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW).
• Demonstrated knowledge and understanding of trauma-informed care principles.
• Proficient in cultural competence to effectively engage with diverse client backgrounds.
• Strong communication skills, both verbal and written, are essential for successful interactions with clients and community partners.

• Previous experience within a social service agency or healthcare setting, particularly with hospice or palliative care, is preferred.

The salary for this position ranges from $60,000 to $100,000 annually, commensurate with experience, plus a sign-on bonus.
