Quality Assurance Nurse (RN or LPN)
Location: Flagstaff, AZ
Schedule: Flexible | Mon-Fri 7:00 AM-3:00 PM, 8:00 AM-4:00 PM, or 8:00 AM-5:00 PM | Occasional weekends
Overview:
We are seeking an RN or LPN to support Quality Assurance, Staff Education, and Infection Prevention. This role ensures regulatory compliance, drives quality improvement, and enhances resident care through audits, training, and infection control oversight.
Responsibilities:
  • Conduct QA audits and monitor compliance with policies
  • Review CASPER reports and develop QAPI improvement plans
  • Coordinate survey readiness and corrective action plans
  • Lead QA and Infection Control Committee meetings
  • Oversee infection prevention per CMS/CDC guidelines
  • Develop and deliver staff education and competency programs

Qualifications:
  • Active AZ RN or LPN license
  • DPS Fingerprint Clearance Card
  • Infection Preventionist Certification
  • Valid AZ driver's license (travel required)
