Job Title:
Hospice Clinical Liaison RN

Location: Evergreen Park/Oak Lawn, IL.

Territory Coverage/Travel Requirements:
This role requires travel throughout the south side of Chicago and surrounding south suburbs based on patient referrals and business needs. Coverage areas may include hospitals and healthcare systems such as The University of Chicago Medicine, University of Illinois Hospital & Health Sciences System, Northwestern Medicine Palos Hospital, and Advocate Christ Medical Center, among others within the service area.

Overview

JourneyCare Hospice is seeking a Registered Nurse (RN) to serve as a Hospice Clinical Liaison supporting hospital-based referrals.

This role is ideal for RNs with hospital or hospice experience who are interested in care transitions, patient advocacy, and working closely with interdisciplinary hospital teams. Hospice or liaison experience is helpful but not required—we provide training for strong acute care nurses transitioning into hospice.


Schedule
  • Schedule Options: Monday–Friday 9AM – 5:30PM OR Monday-Friday 10AM - 6PM
  • NO NIGHTS
  • One Weekend every 6-8 weeks

Responsibilities
  • Perform hospice eligibility assessments and bedside clinical evaluations in hospital settings
  • Support hospice admissions and complete required documentation (CTI, consents)
  • Collaborate with physicians, case managers, social workers, and discharge planners
  • Assist in determining hospice eligibility and appropriate level of care
  • Coordinate transitions from hospital to hospice services
  • Educate hospital staff and referral sources on hospice services and admission process
  • Build and maintain strong relationships with hospital partners
  • Support urgent and General Inpatient (GIP) hospice referrals when needed

Qualifications
  • Active Registered Nurse (RN) license in Illinois
  • Graduate of an accredited nursing program
  • 1–2+ years of RN experience (hospital or hospice experience preferred)
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced hospital environment
  • Strong clinical judgment and organizational skills
  • Current CPR certification
  • Valid driver’s license and insurance

Preferred Experience
  • ICU, med-surg, oncology, emergency department, or case management background
  • Hospice or palliative care experience a plus
  • Experience with care transitions or discharge planning
  • Exposure to patient education or family discussions in acute care settings
