Now Hiring: PRN and Full-Time Home Health Registered Nurses (RN)
West and Central Valley | Senior Living Communities
Covenant Home Health is seeking compassionate and skilled Registered Nurses (RNs) to deliver high-quality, resident-centered care across senior living communities in the West and Central Valley regions. This role offers a unique opportunity to practice primarily within established campuses, providing consistent, personalized care in a collaborative and supportive environment.
As a Home Health RN, you will assess, coordinate, and manage patient care within senior living communities and select private residences. In partnership with Fellowship Square, Christian Care, our team is committed to delivering compassionate care aligned with a shared mission of serving seniors with dignity and excellence.
Approximately 90% of care is provided on senior living campuses, where you will work closely with an interdisciplinary team to develop and implement individualized care plans that meet each patient's unique needs.
Responsibilities
  • Case Management: Develop and implement individualized care plans for patients, coordinating with physicians, family members, and other healthcare providers to ensure optimal care.
  • Patient Assessment: Conduct thorough assessments of patients' physical, emotional, and social needs and update care plans as required.
  • Care Coordination: Ensure the timely delivery of care services, including scheduling visits and coordinating with other healthcare professionals.
  • Medication Management: Oversee medication administration, monitor for side effects, and educate patients and families on proper medication use.
  • Patient Education: Provide education on health management, disease prevention, and improving quality of life for patients and their families.
  • Documentation: Maintain accurate and up-to-date records for all patient visits, including care plans, progress notes, and communication with the care team.
  • Advocacy: Act as an advocate for the patient's needs, ensuring that care is delivered in accordance with the patient's preferences and goals.
  • Collaboration: Work closely with physicians, therapists, and other care team members to ensure a coordinated approach to patient care.
Qualifications
  • Active Registered Nurse (RN) licensed in Arizona with current valid credentials.
  • Minimum two (2) years' Registered Nursing experience in Home Health required.
  • Must pass background check and meet all health and safety requirements.
  • Valid driver's license and reliable transportation required.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
