Job description

Position: Nurse Practitioner – Nursing Home or House Call
Location: NYC Boroughs 
Schedule: Full-Time or Part-Time (Flexible, including weekends only)

About the Opportunity
A large, well-established multispecialty group with locations throughout the Greater NYC area is seeking  Nurse Practitioners to provide care in either nursing homes or through house calls. Candidates may choose the borough that works best for them, with current needs in the Bronx for house calls and in Manhattan for nursing home care. This permanent role offers flexibility, strong clinical support, and the ability to tailor your schedule to your needs.

Spanish language skills are preferred but not required. The group can sponsor H1B visas if needed. For house calls, a car service will be provided.

Key Responsibilities

  • Provide comprehensive medical care in either nursing home settings or through house calls

  • Evaluate, diagnose, and manage acute and chronic conditions

  • Order and interpret diagnostic tests, develop treatment plans, and prescribe medications

  • Work collaboratively with physicians, other APPs, and support staff to ensure high-quality care

  • Maintain accurate documentation in EMR

  • Educate patients and caregivers on preventive care, treatment, and follow-up
