Area of Responsibilities
Make home visits on patients for the purpose of Admissions, Post-hospital, Recertification, skilled nursing visits, Aide supervisory visits and/or discharge visits, as assigned.
Meets or exceeds productivity goals in accordance with Agency established 70th percentile.
Proficient in use of Maestro software to navigate patient record, enter orders, initiate and implement plan of care, and complete Agency required documentation for all visit types within 48 hour time period.
Provides ongoing communication with appropriate staff members to make known any changes in patient needs, schedules, physician orders, special instructions and patient updates.
Verifies the Plan of Care prior to each visit and provides care according to physician's orders and Agency policies and procedures.
Adheres to all Infection Control guidelines in accordance with Agency policies and procedures.
Appropriately updates/revises Medication Profiles and Home Health Aide care plans as assigned per agency policy.
Assesses, plans, implements and evaluates patients on every visit and documents in compliance with Agency policy/procedure.
Assist in ensuring that the patient home folder is kept accurate and current.
Assists in orientation of new RN/LPN staff members and preceptor to nursing students, as requested.
Attends Reassessment/Case Conferences regarding patients and provides input for care planning and reassessment.
Charges for any equipment and supplies for patient care are documented for appropriate reimbursement.
Communicates any changes in a patient's status and/or Plan of Care to the Client Care Supervisor.
Compares/verifies actual care given (as ascertained on Aide supervisory visit) by Home Health Aide to current Home Health Aide Care Plan, documents visit on supervisory note and reports any discrepancies.
Completes any QA requests within 48hours of receipt.
Demonstrates maintenance of confidentiality of patient information.
Develops and implements discharge plans and provides appropriate education and discharge instructions to patients, upon discharge, in accordance with Agency policy and procedure.
Ensures that all clinical notes, supply/visit logs, etc. are accurate and submitted per Agency policy.
Implements and documents appropriate nursing interventions to meet patient needs and changed conditions.
Must perform intermittent visits on patients receiving care from LPN's , as assigned.
Participates and maintains the Agency philosophy, mission statement and administrative policies and procedures.
Participation in in-services and/or continuing education programs is consistently demonstrated.
Prepare appropriate documentation for mileage, visits and/or time and submits to the Client Care Supervisor on a timely basis.
Provides nursing care requiring specialized skills, per current nursing practice and Agency policies and procedures.
Remains available and is able to be located during all assigned working or 'on call' hours.
Rotates 'on-call' as assigned for weekends and holidays.
Writes verbal orders for initial and any changes to patient Plan of Care (including PRN visits) on the day of the identified change and/or visit.
Compliance Statement
Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.
