Job Summary:

A professional registered nurse who plans, manages, coordinates, implements, and evaluates the ongoing care and psychosocial needs of the patient.

Essential Job Functions:

  1. The case manager is responsible for ongoing monitoring and evaluation of patient outcomes and for the

     provision of quality, cost- effective and medically appropriate services.

  1. Collaborates and assist members of the health care team, the patient and the family to determine home needs

     for effective, efficient, and appropriate discharge care

  1. On-call rotation for discharge planning after 5 pm and on weekends
