Description

The Hospice Registered Nurse (RN) provides compassionate, patient-centered care for individuals with terminal illnesses and their families. As a key member of the hospice team, you will deliver quality end-of-life care in a supportive and empathetic manner, focusing on comfort, symptom management, and emotional support. You will work collaboratively with physicians, social workers, chaplains, and other healthcare professionals to ensure a dignified and peaceful transition for patients.


Schedule: PRN, Part-Time, Weekends, On-Call, 8 Hour Shifts, Saturday & Sunday


Primary Responsibilities and Duties:

  • Conduct thorough assessments of patients; physical, emotional, and spiritual needs.
  • Develop and implement individualized care plans in collaboration with the patient, family, and other hospice staff.
  • Administer medications, manage pain and symptom control, and perform medical procedures as needed.
  • Monitor and document patient progress, changes in condition, and response to treatments.
  • Educate patients and their families on disease progression, care options, and symptom management.
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care.
  • Offer emotional and psychological support to patients and families, addressing their concerns and facilitating discussions about end-of life care.
  • Coordinate with other members of the hospice team to ensure comprehensive and continuous care.
  • Advocate for patients' preferences and needs, respecting their values and wishes.
  • Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards.
  • Participate in on-call rotation as required to provide care and support after hours.
  • Perform all other duties assigned.

Requirements

Job Qualifications:

  • Valid license as a Registered Nurse (RN) in the state where work is performed, Ohio.
  • 1 or more years of nursing experience, preferably with a focus on hospice, palliative care, or related fields.
  • Strong clinical skills and knowledge of nursing principles, procedures, and best practices in long-term care.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, caregivers, and healthcare professionals.
  • Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
  • Understanding of regulatory requirements in hospice settings and nursing practices.
  • Ability to build rapport with patients through a compassionate and caring demeanor.

Physical Requirements:

  • Ability to lift and transfer patients, with or without assistance, up to 50lbs.
  • Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods.
  • Strength to handle medical equipment and assist patients with mobility and physical activities.
  • Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling delicate medical instruments.
  • Ability to respond swiftly and efficiently to emergency situations.
  • Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • The job requires accepting criticism and dealing calmly and effectively with high-stress situations.
  • The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
  • The ability to keep your hand and arm steady while moving your arm or while holding your arm and hand in one position.
