Position Description: We are looking for a dedicated and compassionate Nurse Practitioner ( NP ) for our Skilled Nursing Facility ( SNF ) in Springfield, MO for bedside patient care. The NP will serve as the primary medical provider for assigned residents, working closely with the Medical Director, facility staff, and interdisciplinary teams. This is a full-time, in-person position Monday through Friday 9am to 5pm. No weekends, no nights, no on-call.

Qualifications of the Nurse Practitioner ( NP ):


  • Valid Nurse Practitioner ( NP ) license in the state of Missouri, in good standing

  • Active DEA registration

  • 1+ years of experience as a Nurse Practitioner; SNF, geriatric, or post-acute experience preferred

  • Strong clinical documentation skills and familiarity with EMR systems

  • Excellent communication skills with patients, families, and interdisciplinary teams

Responsibilities of the Nurse Practitioner ( NP ):


  • Upon admission, examine residents and review their medical records to establish baseline assessments

  • Conduct routine follow-up visits and manage chronic conditions including diabetes, hypertension, CHF, COPD, and dementia

  • Prescribe and adjust medications including controlled substances per state regulations

  • Lead transitional care management and discharge planning with social work, nursing, and rehabilitation teams

  • Engage with residents' families on care plans, goals of care, and advance directives
