Job Title:Bereavement Coordinator

Job Location:Morristown, TN

Job Shift: Part Time

POSITIONSUMMARY:The Bereavement Coordinatorprovidessupportive services to help meet the needs of the terminally illhospice patientsand familiesas needed.Assistsandsupportto familiesduring theirbereavement.Worksas a member of thehospice team in providingcomprehensive end-of-lifecare. Responsible for developing, implementing,andsupervising the bereavement program and for the delivery of relatedservices.

ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

  • Managesthe bereavement services program utilizing professional staff and volunteers.

  • Overseesadequacy and appropriateness of bereavement programs for patientsand family members.

  • Developsnew bereavement programs and services as needed.

  • Developseducational programs and materials for patients,families, staff,and the community regarding loss, grief,and coping with bereavement.

  • Assistsagencyleadershipin educational training program.

  • Designsmaterials for distribution to families eligible for and/or receivinghospice bereavement services.

  • Plans, implements,and supervisesbereavement group events.

  • Supervisessupport staff involved in bereavement program.

  • Overseesbereavement follow-up by patient care staff and volunteers.

  • Overseesthe volunteer component of the bereavement services.

  • Conductsthe bereavement section of the IDG conference.

  • Participatesin the maintenance of the bereavement component of the community and staff resource libraries.

  • Participatesin the orientation and training of new employees and volunteers working in thehospice program.

  • Preparesreports as requested byagency leadership.

  • AttendsInterdisciplinary Group(IDG)conferencesasrequired.

  • Works with IDG concept of patient care.

  • Other duties as assigned byagency leadership.

QUALIFICATIONS&REQUIREMENTS FOR THE POSITION:

  • Graduate from an accredited institution of social work, preferred.

  • At least two (2) years of experience in a healthcare or hospice setting, preferred.

  • Musthave experience or education in grief or loss counseling. Active patient contact within past three(3)years, preferred.

  • The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.

  • LCSW preferred

PREFERRED KNOWLEDGE AND SKILLS:

  • Must have strong computer skills to utilize Microsoft Outlook andagencysystems.

  • Self-motivated; able to work independently with minimal oversight.

  • Demonstrated ability to supervise and directothers as needed.

  • Ability to respond appropriately to crisisandimmediate patent/family needs.

  • Acceptance of and adaptability to different social, racial, cultural and religious modes.

  • None.
