Position Summary

The Per Diem Hospice RN provides skilled nursing care, patient and family education, and end-of-life support in the patient’s home or facility. This role is ideal for a nurse who is flexible, dependable, and passionate about delivering comfort-focused care. Experience working with patients on ventilators is highly preferred.

Key Responsibilities

Provide comprehensive nursing assessments and develop individualized hospice care plans.

Perform scheduled and PRN visits, including routine, supervisory, and symptom-management visits.

Manage symptoms such as pain, respiratory distress, agitation, and nausea.

Provide specialized care for patients on ventilators, including assessment, monitoring, and caregiver education related to respiratory equipment and comfort.

Educate patients and caregivers on disease progression, medications, safety, and comfort measures.

Collaborate with the interdisciplinary team (IDG) including MD, social worker, chaplain, LVN, HHA, and others.

Document all care in accordance with Medicare, state, and company standards.

Communicate changes in patient condition promptly to the clinical team.

Participate in on-call rotation as needed (optional for some per-diem roles depending on agency needs).

Qualifications

Active California RN license in good standing.

Minimum 1 year of nursing experience; hospice or home health experience preferred.

Experience caring for patients on ventilators strongly preferred.

Reliable transportation, valid CA driver's license, and auto insurance.

Strong communication, assessment, and critical-thinking skills.

Ability to work independently in home settings.

EMR experience preferred (training provided).
