Job description

All Care is seeking a Hospice Registered Nurse (RN) to provide skilled, compassionate nursing care to patients with life-limiting illnesses in their homes. The Hospice RN is responsible for case management, symptom control, patient and family education, and coordination of care with the interdisciplinary hospice team to ensure comfort, dignity, and quality of life.

Schedule: Monday-Friday

Work Location: On the road in assigned territory, including Boston and surrounding communities.

Position Type : Full Time

  • Perform comprehensive initial and ongoing nursing assessments of hospice patients in the home
  • Develop, implement, and update individualized plans of care in collaboration with the hospice interdisciplinary team
  • Manage pain and other symptoms
  • Administer medications, treatments, and interventions as ordered
  • Provide patient and caregiver education on disease progression, medications, and end-of-life care
  • Coordinate care with physicians, nurse practitioners, social workers, chaplains, and other team members
  • Serve as the primary case manager for assigned patients
  • Monitor patient condition and communicate significant changes to the care team
  • Participate in interdisciplinary team (IDT) meetings and care planning
  • Provide emotional support to patients and families during end-of-life care
  • Complete accurate, timely, and compliant documentation in EMR systems
  • Ensure compliance with hospice regulations, policies, and Medicare Conditions of Participation
Qualifications :
  • Valid driver’s license and reliable transportation
  • Active Massachusetts Registered Nurse (RN) license is required
  • Minimum of 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
  • Strong clinical assessment and symptom management skills
  • Knowledge of end-of-life care practices
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage a caseload
  • Proficiency in using electronic health record (EHR) systems for documentation and communication
