WEEKEND REGISTERED NURSE RN – HOME HEALTH

$7,500 Sign-On Bonus

Looking for a 3-day work week with full-time benefits?

This Weekend RN opportunity offers a consistent schedule with flexibility built in.

SCHEDULE

• 12-hour shifts

• Saturday and Sunday required

• One weekday of your choice

• Full-Time status

LOCATION

• St. Joseph and Elkhart Counties

• Must be willing to travel within coverage area

As a Home Health RN with Elara Caring, you provide one-on-one care in the patient’s home. You manage your own caseload, build meaningful relationships, and see real progress in real time.

WHAT YOU WILL DO

• Manage and coordinate your own home health caseload

• Complete Start of Care, follow-up, and discharge visits

• Perform comprehensive patient assessments

• Develop and update individualized plans of care

• Complete OASIS documentation accurately and timely

• Educate patients and families on medications and disease management

• Monitor patient progress and report changes to physicians

• Collaborate with interdisciplinary care team members

• Ensure documentation meets compliance and regulatory standards

WHAT YOU NEED

• Active, unrestricted RN license

• At least 1 year of nursing experience

• Willingness to travel within the coverage area

• Reliable vehicle, valid driver’s license, and auto insurance

• Home Health experience preferred

• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
