Job description

As a Registered Nurse (RN), you’ll play a vital role in providing exceptional care and support to patients in the comfort of their homes. Working alongside an interdisciplinary team, you’ll help patients achieve the highest level of comfort, independence, and quality of life while supporting families through every step of their care journey.

What You'll Do
  • Conduct comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
  • Develop, implement, and evaluate individualized care plans in collaboration with physicians, therapists, social workers, aides, and other healthcare professionals.
  • Deliver skilled nursing care, including:
    • Wound care
    • Medication management
    • IV therapy
    • Symptom and pain management
    • Patient and caregiver education
  • Provide compassionate education, guidance, and emotional support to patients and their families.
  • Supervise and support LPNs/LVNs, home health aides, and other care team members.
  • Coordinate patient care activities, visit scheduling, and documentation to ensure quality and compliance.
  • Monitor patient progress, respond to changes in condition, and modify care plans as needed.
  • Participate in admissions, urgent patient needs, and on-call rotations as required.
  • Maintain accurate clinical documentation and uphold agency policies, professional standards, and regulatory requirements.

Requirements

Required Qualifications
  • Graduate of an approved school of professional nursing.
  • Current Registered Nurse (RN) license in the state of agency operation.
  • Minimum of one (1) year of RN experience.
  • Current CPR certification (or ability to obtain and maintain upon hire).
Additional Requirements
  • Valid driver’s license.
  • Reliable, insured vehicle in good working condition.
  • Ability to travel to patient homes and business locations as needed.
