Job Description
You'll work within a structured behavioral health environment supporting treatment, rehabilitation, and long-term patient outcomes for an adult patient population in a state correctional healthcare setting.
You'll work independently within your scope of practice while collaborating with treatment teams, internal departments, and community support agencies to coordinate patient care and long-term treatment outcomes.
Work Schedule: Monday-Friday / 8:00 AM-4:00 PM / Rotating weekends asneeded
Key Responsibilities
  • Develop, coordinate, and update individualized behavioral health treatment plans based on patient needs and progress.
  • Conduct assessments, evaluations, admissions coordination, and crisis intervention within scope of practice.
  • Coordinate referrals and collaborate with interdisciplinary teams, internal departments, community agencies, and external partners to support continuity of care.
  • Evaluate treatment effectiveness, modify interventions, and maintain accurate documentation and reporting.
  • Participate in treatment team meetings, training, and program improvement activities while supporting lower-level providers when appropriate.

Qualifications
Requirements
  • Master's degree.
  • Current BLS from the American Heart Association or American Red Cross.
  • Current active Ohio Licensed Independent Social Worker (LISW) license required and maintained in accordance with state continuing education requirements.
  • Ability to work with patients who may have behavioral or psychiatric conditions and meet role-related physical requirements, including occasional lifting up to 60+ lbs.
