Job description

Family Home Health Network, is a provider of intermittent Home Health care, Palliative care, Hospice care and Skilled/Custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the facility/home environment to determine specific care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

THIS POSITION IS EVERY OTHER WEEKEND SATURDAY AND SUNDAY 7AM TO 7PM. 

Qualifications:

  • Must possess current Illinois RN licensure

  • Possess a minimum of two (2) years experience (Hospice Preferred)

  • Current CPR

  • Valid IL driver’s license and auto insurance policy

  • HCHB EMR experience a plus!

  • Must possess excellent clinical nursing and customer service skills
