Job Summary:
A professional registered nurse who plans, manages, coordinates, implements, and evaluates the ongoing care and psychosocial needs of the patient.
Essential Job Functions:
  • The case manager is responsible for ongoing monitoring and evaluation of patient outcomes and for the provision of quality, cost- effective and medically appropriate services.
  • Collaborates and assist members of the health care team, the patient and the family to determine home needs for effective, efficient, and appropriate discharge care.
  • On-call rotation for discharge planning after 5 pm and on weekends

    MINIMUM QUALIFICATIONS:
    Current RN License in the state of Texas.
    Two (2) years clinical experience in hospital setting.
    BLS required.
